To Whom It May Concern:Pursuant to the New York Freedom of Information Law, I hereby request the following records:Per the NYS Information Security Breach and Notification Act, "State entities and persons or businesses conducting business who own or license computerized data which includes private information must disclose any breach of the data to New York residents whose private information was exposed."This is a request for all of the registered business entity names of entities which have reported Information Security Breaches to the State of New York per the guidelines established in the NYS Information Security Breach and Notification Act, as well as for information regarding the nature and scope of these breaches (where disclosure of such information would neither interfere with a law enforcement investigation, nor jeopardize the capacity of an agency or an entity that has shared information with an agency to guarantee the security of its information technology assets, such assets encompassing both electronic information systems and infrastructures). I would like this information for all breaches which have occurred in the last three (3) years.Please note: I am submitting this request in affiliation with a private corporation, seeking to use this information for the company’s business, for commercial purposes.The requested documents will be made available to the general public, and this request is not being made for commercial purposes.In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.Thank you in advance for your anticipated cooperation in this matter.
