Pursuant to the New York State Freedom of Information Law (1977 N.Y. Laws ch. 933), I hereby request the following records:
Copies of all survey responses collected by the Metropolitan Transit Authority through the agency’s Customer Survey Initiative. The survey program is described here:

The requested records include all demographic data collected by the MTA for each survey taken. This includes race, ethnicity, gender, age, salary, language, and so forth. In the interest of complying with with the Freedom of Information Law’s privacy requirements, please redact or withhold any information that would identify individuals who took and completed the survey.
The requested records include the raw survey data as well as any reports generated within the MTA from that data. Please provide the raw data in machine-readable form, such a CSV or Excel file.
Please limit this request to data collected from surveys performed by the MTA between January 1 2014 and June 1 2016.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.
