Pursuant to Executive Law §94(19)(a) and the Commission regulations, I hereby request the following records:
Financial Disclosure Statements filed for FY 2017 for following staff of the NY State Senate (Office of Senator Martin J. Golden):
• Steven A. Taylor
• Gerard Kassar
• John Quaglione
• John Seravalli
Additionally, for the aforementioned and time period, any records concerning:
• Notices of Financial Disclosure Statement filing delinquencies;
• Notices of civil assessments, or settlement agreements;
• Documents required to be disclosed pursuant to Legislative Law Article 1-a;
• Substantial Basis Investigation Reports issued by the Commission.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
