Request for Commission records

To Whom It May Concern:

In accordance with Executive Law §94(19)(a) and Commission regulations for access to publicly-available records:

Please send all Commission records pertaining to Anthony Testaverde, received calendar year 2008 or later, in relation to employment by the NYS Legislature.

Records may include Financial Disclosure Statements; notices of civil assessment or settlement agreement; documents required to be disclosed pursuant to Legislative Law Article 1-a (the Lobbying Act); and Substantial Basis Investigation Reports issued by the Commission pursuant to Executive Law §94(14-a) and (14-b).

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
