Pursuant to the New York Freedom of Information Law, I hereby request the following records:

1. Any and all New York State Public Health Law(s) that regulates and/or requires and/or mandates COVID-19 immunization (starting January 2019 until now) as a condition of employment and/or maintaining an employment in either private and/or public setting such as(but not limited to): hospitals, nursing homes, doctors’ offices, schools, colleges, other institutions of higher learning, transportation, public government offices, police department, fire department, health department, sanitation department, any type of private business/venue and/or public space (i.e.. restaurants, grocery stores, coffee shops) and/or any other business in public and/or private entity serving the general public in New York City (NYC) and New York State (NYS).

2. Any and all New York State Public Health Law(s) that regulates and/or requires and/or mandates COVID-19 immunization (starting January 2019 until now) as a condition of education and maintaining education in either/or private/public sector (pre-K through college) in both NYC and NYS

3. Any and all New York State Public Health Law(s) that regulates and/or requires and/or mandates COVID-19 immunization (starting January 2019 until now) as condition of attendance of any school/college (pre-K though college) related public events such as, but not limited to, graduation, prom, ice cream sales, theatre performances, afterschool clubs in both NYC and NYS

4.Any and all New York State Public Health Law(s) that regulates and/or requires and/or mandates COVID-19 testing (starting January 2019 until now) as a condition of employment and/or maintaining an employment in either private and/or public setting such as(but not limited to): hospitals, nursing homes, doctors’ offices, schools, colleges, other institutions of higher learning, transportation, public government offices, police department, fire department, health department, sanitation department, any type of private business/venue and/or public space (i.e.. restaurant, grocery store, coffee shop) and/or any other business in public and/or private entity serving the general public in New York City (NYC) and New York State (NYS).

5.Any and all New York State Public Health Law(s) that regulates and/or requires and/or mandates COVID-19 testing (starting January 2019 until now) as a condition of education and maintaining education in either/or private/public sector (pre-K through college)

6. Any and all New York State Public Health Law(s) that regulates and/or requires and/or mandates COVID-19 testing (starting January 2019 until now) as condition of attendance of any school/college (pre-K though college) related public events such as, but not limited to, graduation, prom, ice cream sales, theatre performances, afterschool clubs.

7.Any and all New York State Public Health Law(s) that regulates and/or requires and/or mandates COVID-19 masking (starting January 2019 until now) as a condition of employment and/or maintaining an employment in either private and/or public setting such as(but not limited to): hospitals ,nursing homes, doctors’ offices, schools, colleges, other institutions of higher learning, transportation, public government offices, police department, fire department, health department, sanitation department, any type of private business/venue and/or public space (i.e.. restaurant, grocery store, coffee shop) and/or any other business in public and/or private entity serving the general public in New York City (NYC) and New York State (NYS).

8. Any and all New York State Public Health Law(s) that regulates and/or requires and/or mandates COVID-19 masking (starting January 2019 until now) as a condition of education and maintaining education in either/or private/public sector (pre-K through college)

9. Any and all New York State Public Health Law(s) that regulates and/or requires and/or mandates COVID-19 masking (starting January 2019 until now) as condition of attendance of any school/college (pre-K though college) related public events such as, but not limited to, graduation, prom, ice cream sales, theatre performances, afterschool clubs.

10. Any other Files, Records, Papers or Documents required by Law to be Made Available for Public Inspection and Copying
