Pursuant to the New York State Freedom of Information Law (1977 N.Y. Laws ch. 933), I hereby request the following records:
A Log of all Freedom of Information Law ("FOIL") requests submitted to the agency, dating from at least March 5, 2015 to the date this request is processed. This log will likely feature--at minimum--the date of request, the requester's name, the requester's organizational affiliation (if applicable), a description of the request and whether or not the request was ultimately fulfilled.
For reference or to further assist your search for responsive records, I can advise you that the agency previously fulfilled a FOIL request of an effectively equivalent wording on or around April 15, 2015. The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.
