New Jersey Poll Worker Reports

To Whom It May Concern:

Pursuant to Open Public Records Act ("OPRA"), I hereby request the following records:

I request all logs and reports filed by New Jersey poll workers for the following 2014 elections:
(i) The primary election held on June 3, 2014. (ii) The U.S. House special election held on June 3, 2014. (iii) The general election held on November 14, 2014. (iv) The U.S. House special election held on November 14, 2014.

The logs and reports should include: any complaint forms filed by voters at their polling location, any incident reports of voting machine malfunction including documentation of poll worker resolutions or contracted repair, any emergency ballot box control logs.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 7 business days, as the statute requires.
