Pursuant to the New Hampshire "Access to Government Records and Meetings" Statute (RSA Ch. 91-A), I hereby request the following records:

The data record layout, data frame, or any equivalent data framework used by this agency to document maternal deaths. Materials responsive to this portion of this request should include reference to the data elements collected by this department and should not include any personal information. Please feel free to redact any personally-identifying information.

Your 91-A request for public records described as the data record layout, data frame, or any equivalent data framework used [by NH DHHS] to document maternal deaths has been forwarded to me for response.

Your request is for "The data record layout, data frame, or any equivalent data framework used by this agency to document maternal deaths." As there is no public record responsive to your request, I am denying your request in part and seeking clarification.

By way of further information, the NH Division of Public Health Services gathers information relative to maternal deaths from three sources, hospitals, the NH Medical Examiner and from the Bureau of Vital Records. Information is gathered manually from hospitals and the Medical examiner. Data is gathered electronically from the vital records database. At this point I have not yet determined whether there exists a data record layout relative to vital records data that is responsive to your request. I anticipate I will have that information on or before November 3, 2017. Can you please clarify whether the data record layout (if any) for data exchange between the NH Division of Public Health and the Bureau of Vital Records is the public record you are seeking?

NH determines maternal deaths in accordance with NH RSA 132 (see attached link). Specifically, NH RSA 132:29 establishes the Maternal Mortality Review Panel. http://www.gencourt.state.nh.us/rsa/html/X/132/132-mrg.htm

In addition, I have attached the most recent report on maternal mortality in NH.
