1. Full name of each sworn police officer

2. Current badge number

3. Current rank

4. Gender

5. Race

6. Date of employment

7. Current assignment/police beat

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 7 business days, as the statute requires.

In order for this to be an official request, you will need to complete a 720 form found on our web site. Once you have completed the 720 form, please send that back along with a copy of your Tennessee photo ID.

Processing will not begin until we have received both the completed 720 form and a copy of your Tennessee photo ID.
