Pursuant to the Alabama Public Records, I hereby request the following records:

(1) any and all data on the number of wasted, spoiled or expired COVID-19 vaccine doses from 12/11/2020 to 01/26/2021, and

(2) any and all data on the total number of COVID-19 vaccine doses administered from 12/11/2020 to 01/26/2021.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you for the submission of your request to the Alabama
Department of Public Health.

There is no data in the Vaccine Ordering Allocation System about
COVID-19 vaccine loss from December 11, 2020 to January 26, 2021. News
releases from the Department repeatedly state that no vaccine has been
wasted. Likewise, the Department has not received any reports of expired
vaccine. The Department has received one report of 600 doses of vaccine
that were spoiled because of a mechanical issue with a facility's
freezer. The Department is also aware that vaccine was spoiled in transit
to two facilities because it was not maintained in the proper temperature
range during transport; however, that vaccine was replaced by
Moderna/McKesson.

The ADPH website has information concerning the doses of COVID-19
vaccine allocated, administered and delivered at
https://alpublichealth.maps.arcgis.com/apps/opsdashboard/index.html#/e4a232feb1344ce0afd9ac162f3ac4ba
. There is also a second tab on that page that displays locations of
vaccine providers and a third tab that provides explanations of the data
included on the dashboard.

- I believe that COVID-19 vaccine loss data may be maintain in the ImmPRINT system. Did the Alabama
Department of Public Health search ImmPRINT for the records I am requesting?

- In order to receive COVID-19 vaccines from the federal government, vaccine providers must agree to "report COVID-19 vaccines and diluents that were unused, spoiled, expired, or wasted as required by the jurisdiction’s immunization program" (see U.S. Centers for Disease Control and Prevention, "COVID-19 Vaccination Program Interim Playbook for Jurisdiction Operations," Oct. 29, 2020, p. 21, available at https://www.cdc.gov/vaccines/imz-managers/downloads/COVID-19-Vaccination-Program-Interim_Playbook.pdf). Are you saying that the Alabama
Department of Public Health has received only three such reports? What was the mechanism through which your office received these reports?

- Can you provide me with details, including time, date and location, for the three reports you referenced?
