Pursuant to the state open records law, Ariz. Rev. Stat. Ann. Secs. 39-121 to 39-122 and 39-128, I hereby request the following records:

All logs of inmate grievances and associated grievances (if available), including but not limited to all those submitted via Inmate Grievance form and recorded using the Unit Coordinator Grievance Log, Form 802-9. Please include all available segregable data, including but not limited to the date on which the grievance was submitted, the nature of the grievance, any additional appeals or processing involving the grievance, the resolution or status of the grievance, etc.

• Eloy Detention Center
• Florence Correctional Center
• La Palma Correctional Center
• Red Rock Correctional Center
• Saguaro Correctional Facility

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
