This is a request under the Freedom of Information Act. I hereby request the following records:

All FDA records for all New Dietary Ingredient notifications and submissions, plus the FDA response (if any) whether in opposition or otherwise, plus any additional correspondence between the FDA and the company/person filing the notification, all inclusive, during 1995.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.

I want to give you an update on your NDI FOIA requests and ask if it would be possible for me to combine your requests into one. The below requests are all dealing with them same content (NDI's) and it would be much easier for me to track if I could have all the years (1995 - 2015) under one tracking number. I will continue to process each year and send to you, I would just track them under request #2016-905 (1995-2015). I am currently working on completing years 2006 & 2002 and should have those years out to you in the next couple of weeks.
