Pursuant to the California Public Records Act, I hereby request the following records:

Informational copies of all American Indian and Alaska Native death certificates held by the Santa Clara County Clerk-Recorder's office.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 10 calendar days, as the statute requires.

In response to your California Public Records Act Request e-mail dated October 7, 2020, this e-mail is to inform you that the County Clerk-Recorder's Office does not have an index within our system for the information requested.

Additionally, Please note that the requested death record information is considered a Vital Record to which access is restricted, per the Health & Safety Code §102230, "...all comprehensive birth, death, and marriage record indices prepared or maintained by local registrars and county recorders shall be kept confidential and shall be exempt from disclosure under the California Public Records Act."

If there is a specific death record(s) you wish to obtain, the restricted records may be obtained by completing the attached Death Certificate Request Form. In filling out the application, please provide all the information you have available. The fee for searching for a death certificate is $21.00 per search and falls under existing services of this office; the fees associated with this service are approved by our Board of Supervisors pursuant to Government Code §27366 (approved fee schedule attached). Pursuant to Health & Safety Code §103650, that fee must be paid in advance. You may complete your request via mail, one of two drop boxes located on-site (70 West Hedding St., San Jose, CA 95110), or online using www.vitalchek.com<http://www.vitalchek.com>.

Your request has been forwarded to the Public Health Department as they may be better able to assist you with your request.
