From: Joe Yerardi

To Whom It May Concern:

Pursuant to the California Public Records Act (California Government Code §§ 6250), I hereby request the following records:

1) Aggregated, monthly summary of new and renewed medical marijuana card applications since the inception of the program through August 31, 2015.

2) Any available data on the ages of medical marijuana patients (i.e. the number of patients that are 18, the number of patients that are 19, the number of patients that are 20, etc.) as of August 31, 2015.

I would like to receive these records in electronic format—if at all possible. To avoid problems with data compatibility, please provide all electronic information in a comma-delimited or ASCII text file, a format that can be easily opened by any spreadsheet or database program.

I can also accept ASCII text, dbase IV, Microsoft Excel, Microsoft Access and many other formats (just let me know and I'll see if we can use it). I can accept this data on CD-ROM, via email or FTP.

In addition, please provide copies of the record layout, code sheet and all relevant documentation regarding these records.

And if you should determine that portions of the information is exempt from disclosure, please notify me of the reasons for that determination pursuant to Government Code Section 6256.

As some responsive records may be located sooner than others, I request that records be released on a rolling basis, rather than held until all responsive records are located.

Please let me know if there is anything I can do to help clarify or expedite this request. I look forward to hearing from you and appreciate your assistance.

Sincerely,
Joe Yerardi
