Hi Evan I am in receipt of your RSA 91-A Right to Know request relative to Medicaid claims for treatment of Dissociative Identity Disorder. In order to respond to your request I need to know the years of claims that you are seeking. Once I have that information I can likely get you the spreadsheet in three weeks or less.

Hi Mr. Martin,

Thank you for the response. The time frame is between 1/1/2000 through 2017. I am hoping that the spreadsheet can include provider information (such as NPI), service provided (e.g., CPT code), date or year of service, and amount reimbursed. Please let me know if you have any further questions.

Best,
Evan
