The data record layout, data frame, or any equivalent data framework used by this agency to document maternal deaths. Materials responsive to this portion of this request should include reference to the data elements collected by this department and should not include any personal information. Please feel free to redact any personally-identifying information.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Dear Ms. Lustig,

This email is in response to your recent Freedom of Information Act request for information pertaining to the data collected by the Department with respect to maternal deaths.

Records responsive to your request are attached. Just by way of explanation, there are 2 different records regarding the data we collect. The first 4 pages of the attachment is the data we will be collecting in the near future and the remainder of the pages is the data currently being collected, however, please note, we will not have every data piece listed for each maternal death. We attempt to collect as much data as we can, but unfortunately there may be some data elements we do not know for a particular maternal death.

Your request has the hallmarks of a preliminary request in anticipation of a future request for the data itself, however I recommend you contact me or the Department's Communications Manager, Melaney Arnold, who I've also included on this email, prior to making the request. The Illinois FOIA and the statutes governing the use of health data collected by the Department place some restrictions on the release of data, specifically when the data can be used to identify individuals, so we would welcome a discussion with you prior to a request to discuss the data and how your request can be crafted to permit us to release data without concerns for identifying individuals and avoid unnecessary denials or delays in getting the data to you.
