Materials responsive to this portion of this request should include reference to the data elements collected by this department and should not include any personal information. Please feel free to redact any personally-identifying information.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

Good afternoon,
The Ohio Department of Health is in receipt of your request for the "data record layout, data frame or any equivalent data frame network" to record maternal deaths. It is unclear what documents or records you are seeking. Please clarify your request to state what documents you are asking the Department to produce.
The Department does obtain information on maternal deaths that occur in a licensed maternity unit. The form is available on the Department's website at: http://www.odh.ohio.gov/pdf/forms/matinfantdeath.pdf
