This is a request under the Freedom of Information Act. I hereby request the following records:
Copies of 2015 audits of federal awards to the Commonwealth of Massachusetts for the following programs:
1) State Children's Insurance Program
2) Maternal and Child Health Services Block Grant to the States
3) State Medicaid Fraud Control Units
4) State Survey and Certification of Health Care Providers and Suppliers
5) ARRA -- Medical Assistance Program
6) Medical Assistance Program
Summary reports of these audits are available through the Federal Audit Clearinghouse's Image Management System. I would like to clarify that I am requesting copies of the full audit reports.
I also ask that all fees be waived for this request as I am a journalist employed by the Lowell Sun and I intend to use the requested records to publish articles in the public interest and not for any commercial purposes.
I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.
