Pursuant to the Illinois Freedom of Information Act., I hereby request the following records:

I request that a copy of the following documents [or documents containing the following information] be provided to me that pertains to Logan Correctional Center:

Comprehensive list of any deaths in custody from 2020 until the present. This should include corresponding names, dates of death, and causes of death. If possible, please include dates of birth and race information.

In the event that there are fees, please inform me of the total charges in advance of fulfilling my request. I would prefer the request be filled electronically.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
