Pursuant to the New York Freedom of Information Law, I hereby request the following records:

All personnel and disciplinary records concerning Frederick J. Patrick, former Deputy Commissioner of the Department of Corrections, including any record concerning the investigation of his conduct as part of New York City Correction Foundation, Inc.

To assist in the location of records: the investigation was made public in 2003, and uncovered "a scheme to defraud the New York City Correction Foundation, Inc." that included Patrick taking calls of inmates at Rikers Island "of a personal nature."

Prior to his arrest and prosecution, Patrick held the following positions:

January 2002 Deputy Commissioner Community Affairs, NYPD
2001 Commissioner Department of Juvenile Justice
1998 to 2001 Deputy Director, Office of the Criminal Justice Coordinator
1995 to 1998 DOC Deputy Commissioner of Programs
1994 to 1995 DOC Assistant Commissioner Training

These records should be released in the public interest recognized under FOIL, as his case gained public notoriety as a brazen abuse of multiple positions of public trust. Further, Patrick is known to be deceased in 2019 (please see attached PDF), and privacy interests preventing release are now greatly diminished.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
