To Whom It May Concern:

This is a request under the Freedom of Information Act. I hereby request the following records:

All complaints received since January 1, 2000 regarding the "Narconon Arrowhead" drug rehab facility in Oklahoma. This facility is alternately known as Narconon Drug Rehab and/or Narconon Oklahoma, and is listed as being located the following adjacent addresses:

D4152 Road, Canadian, OK 74425
69 Arrowhead Loop, Canadian, OK 74425
Arrowhead Drive, Canadian, OK 74425

I also request that, if appropriate, fees be waived as I believe this request is in the public interest due to the numerous civil lawsuits currently pending against this controversial Scientology-based facility for wrongful deaths, negligence, fraud, etc. Disclosure of the records I am seeking will greatly aid the public's understanding of what is happening at this non-profit organization, and how public service agencies operate when dealing with such a place in regards to how recent complaints related to this facility have been widely reported by local, regional and national media outlets. For examples, please see:

Additionally, the requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, and is not being made for commercial usage.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.

Sincerely,

Ms. R. M. Seibert
