Pursuant to the Illinois Freedom of Information Act., I hereby request the following records:
I hope this request finds you in good spirits.
I request that a copy of the following documents [or documents containing the following information] be provided to me that pertains to Dixon Correctional Center:
♥ Telephone directory of all current Dixon Correctional Center employees and staff.
♥ Email directory of all current Dixon Correctional Center employees and staff.
♥ Comprehensive list of current phone numbers, extensions, and corresponding names. If possible, please include position titles of each extension. For example, extension 3140 is Health Information Associate (position title) and directs the caller to Laurie Koch (name).
In the event that there are fees, please inform me of the total charges in advance of fulfilling my request. I would prefer the request be filled electronically.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
