Pursuant to the New York State Freedom of Information Law (1977 N.Y. Laws ch. 933), I hereby request the following records:
I request information pertaining to the liquor license of the Bay Ridge Manor, indexed as Serial #: 1010903
The Bay Ridge Manor is located 476 76 St, Brooklyn, NY 11209 (Kings County).
My specific request is for:
- Documents related to the original licensing of the Bay Ridge Manor, estimated date 1984
- Documents related to the latest renewal of the Bay Ridge Manor liquor license, effective 11/1/2017
- Documents related to disciplinary proceeding or penalties on account of any violations, dating back to 2010
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
