Dear Data Practices Official,

Pursuant to the Minnesota Government Data Practices Act, I hereby request the following records:

The MP-6946 form pertaining to the February 2nd raid on 1117 S Marquette Avenue Apartment 701 that resulted in the death of Amir Locke.

I am willing to pay applicable fees, however please check with me in advance of proceeding if fees will exceed $25 (twenty five dollars). Please fulfill the request electronically by portal upload or email attachment.
