Pursuant to Michigan's Freedom of Information Act ("FOIA"), I hereby request the following records:
A copy of all autopsy reports performed between 1/1/2000 and the date this request is processed on individuals who died at a Forest View Hospital in Grand Rapids, Michigan.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

Hi Evan,
This office has received your FOIA request to obtain all autopsy reports from 1/1/2000 to present on all deaths that occurred at Forest View Hospital. Unfortunately, this request is impossible to fulfill without the names and dates of death, as we do not have a way to know who died at that facility. Our database isn't set up to know how many deaths occurred at any hospital/facility.
In speaking with the County Legal Counsel, we suggest you send a FOIA request to Forest View Hospital requesting the names and dates of death during that time period, and once you've obtained that information, then send us another request with that information.
