Pursuant to the Texas Public Information Act, I hereby request the following records:
Jail SHUCK SHEET on Richard Coke Mills, from when he was in jail, in solitary, when he had cancer. I need all written instructions to jail staff, all notes, all history of Mr. Mills stay in the McLennan County jail. List of infractions, if any. Date entered the jail, date of release and all JAIL SHUCK SHEET information on Richard Coke Mills.
I am making this a NEW and separate request as I can't seem to get anywhere with the previous one.
PLEASE CHARGE ME WHATEVER AND SEND. THERE IS NO RECORD OF YOU SENDING ME A SHUCK SHEET ON THIS PERSON BEFORE.
Richard Coke Mills, date of birth January 10, 1971. Address, 5134 Lake Shore Drive 76710, approx date of incarceration November 1st. through 10th. 2019.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
