Pursuant to the California Public Records Act (California Government Code §§ 6250), I hereby request the following records:

(1) Copy of all credit card statements for all city issued credit cards between January 1, 2009 to present.

(2) All documents without limitation including contracts, contract amendments, emails, correspondence, invoices or bills concerning ECM Group, Inc. at any time.

(3) All contracts, contract amendments, emails, correspondence, invoices or bills concerning V & M between January 1, 2012 to present.

(4) All City Attorney Contracts and all amendments between January 2009 to present.

(5) All City Attorney resignation letters.

(6) All City Attorney invoices between January 2009 to present.

(7) A copy of the City of Maywood business license for each City Attorney law firm contracted by the City of Maywood between January 2008 to present.

(8) A copy of the City Maywood business licenses for Wildan Group, Inc. between January 2010 to present.

(9) A copy of the City of Maywood business licenses for ECM Group, Inc. between January 2013 to present.

(10) All City Administrator/City Manager/Interim City Manager/administrator contracts and all amendments between January 2009 to present, and all resignation letters.

(11) A copy of all public records act requests made between January 1, 2009 to present

(12) Between January 2015 to present, a copy of all emails from all city email accounts assigned to the following: all city council members, city managers/city administrators, interim city managers/interim city administrators, planning director.

(13) A record of all mileage payments made between January 1, 2013 to present.

(14) A copy of all invoices, bills, or receipts, and payments for catering and entertainment between January 1, 2014 to present, including but not limited to the December 2015 council celebration.

(15) All correspondence to/from the Los Angeles County District Attorney’s office, including but not limited to correspondence related to alleged Brown Act open meeting violations.

(16) A copy of all documents including contracts related to the fair discussed in the May 11th council meeting as reported in the California State Auditors report entitled “City of Maywood: Its Flawed Governance and Financial Mismanagement Could Compromise the Basic Services It Provides to Residents “ (http://www.auditor.ca.gov/reports/2015-803/issues.html).

(17) All documents, including emails, related to the appointment of the “planning director” referred to in the State Auditors report, which may include but are not limited to, notice of personnel action or notice of appointment, signed oaths of office pursuant to Government Code section 3102, new employee orientation or enrollment, correspondence or enrollment with CalPERS.

(18) Any document providing the names of all current City of Maywood employees; and a copy of each employee oath of office pursuant to Government Code section 3102.

(19) All records of ethics training, including but not limited to AB1234 training, of all council members after January 1, 2013.

(20) All correspondence between the City of Maywood and the California State Controller between January 1, 2010 to present.

(21) All correspondence between the City of Maywood and the California Public Employees Retirement System (CalPERS) between January 1, 2010 to present.

(22) All correspondence between the City of Maywood and the Insurance Authority between January 1, 2007 to present.

(23) A copy of all contracts, contract amendments with Republic Services between 2000 to present.

(24) All annual Fair Political Practices Commission disclosure reports for all Council members and city employees filed between January 2009 to present.

(25) A copy of all City contractors/vendors lists between January 2009 to present.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Unfortunately the City of Maywood cancelled the City Clerk’s City Email Address at the direction of Mayor Ramon Medina, Interim City Administrator Reuben Martinez and City Attorney Michael Montgomery. I was advised that the City of Maywood couldn’t afford it and that it couldn’t justify it as a legitimate public expense since an E mail Account wasn’t required for performance of my duties as City Clerk.

I would strongly encourage you to please file a Complaint with the FPPC Fair Political Practices Committee and advised them of the situation. I was just returned my email as this past week on my office computer but denied on my personal phone where I do 98 percent of the City Business.
