Pursuant to the Massachusetts Public Records Law, I hereby request the following records:
(1) any and all data on the number of wasted, spoiled or expired COVID-19 vaccine doses from 12/11/2020 to 01/26/2021, and
(2) any and all data on the total number of COVID-19 vaccine doses administered from 12/11/2020 to 01/26/2021.
I also request that, if appropriate, fees be waived as we believe this request is in the public interest, as suggested but not stipulated by the recommendations of the Massachusetts Supervisor of Public Records. The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, processed by a representative of the news media/press and is made in the process of news gathering and not for commercial usage.
I expect the request to be filled in an accessible format, including for screen readers, which provide text-to-speech for persons unable to read print. Files that are not accessible to screen readers include, for example, .pdf image files as well as physical documents.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 10 business days, as the statute requires.

It was very nice to speak with you this afternoon. DPH continues to process your public records request and a response is forthcoming. In the meantime wanted to send you the information we discussed on the phone today.
(1) any and all data on the number of wasted, spoiled or expired COVID-19 vaccine doses from 12/11/2020 to 01/26/2021
As of February 5, 2021,DPH confirmed that 1272 doses of Moderna and Pfizer COVID-19 vaccines had been reported as wasted.
(2) any and all data on the total number of COVID-19 vaccine doses administered from 12/11/2020 to 01/26/2021.

Thank you for your quick response. It looks like the Department of Public Health provided the same records I'm requesting to The Boston Herald last week:
In particular, this story says, "A total of 1,096 doses of the Moderna vaccine and 176 doses of the Pfizer vaccine were reported as wasted — for a total of 1,272 doses as of Friday, the state Department of Public Health said on Tuesday. The Herald sought the data through a public records request."
Can you please provide me with those same records, or more complete records if they are available?
