Pursuant to the Massachusetts Public Records Law, M.G.L. c.66, §10, I hereby request the following records:

All injury/incident reports for MCI Cedar Junction since Nov. 1, 2013.

I also request that, if appropriate, fees be waived as we believe this request is in the public interest, as suggested but not stipulated by the recommendations of the Massachusetts Supervisor of Public Records. The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, processed by a representative of the news media/press and is made in the process of news gathering and not for commercial usage.

I expect the request to be filled in an accessible format, including for screen readers, which provide text-to-speech for persons unable to read print. Files that are not accessible to screen readers include, for example, .pdf image files as well as physical documents.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 10 business days, as the statute requires.

Are you looking for redacted copies of these files, or data provided in the aggregate?

(Please note that if it is actual reports you seek, they would have to be "hand-pulled" and redacted. The likely format they would be furnished in is PDF format.)

I don't want to stick you guys with a ridiculous amount of redaction work, so if there are hundreds of these reports I would probably narrow my request to a smaller time frame.

Dear Mr. Feathers, your initial request with us was for the Department
of Correction in Massachusetts to provide you with all injury /incident
reports from MCI Cedar Junction for a period of time. Subsequently, you
requested information on the number of assaults and how we defined or
categorized them before you formally asked for redacted copies of all
the injury/incident reports at MCI Cedar Junction.

Please find below, information on assaults at MCI Cedar Junction for
the period you requested. You can let us know what further detailed
information, if any, you may want after reviewing the information
below.

MCI CEDAR JUNCTION ASSAULTS
From November 1, 2013 through February 2014, there were a total of 21
assaults at MCI Cedar Junction.

DOC categorizes assaults as follows: assaults with serious injury,
assaults without serious injury and assaults by throwing substances.
Serious injury requires urgent and immediate medical treatment and
restricts the victim’s usual activity. Medical treatment is more than
mere first aid - e.g., broken bones, stitches, etc.
Assaults by throwing substances include assaults by throwing or
spitting liquid, blood, waste, chemicals, urine, etc. that involved
non-serious injury or no injury. Here is the info requested:

November 2013:

One inmate on staff assault without serious injury

December 2013:

One inmate on inmate assault by throwing substances Seven inmate on
staff assaults without serious injury Three inmate on staff assaults by
throwing substances

January 2014:

Three inmate on staff assaults without serious injury One inmate on
staff assault by throwing substances

February 2014:

Three inmate on staff assaults without serious injury Two inmate on
staff assaults by throwing substances
