Pursuant to the Massachusetts Public Records Law, M.G.L. c.66, §10, I hereby request the following records:

-- Copies of the most recent annual reports filed with the Division of Insurance by approved insurance carriers detailing their policies, procedures, and plan benefits as they relate to parity in the coverage of mental health and substance use disorders.

-- Copies of any notices of violation, reprimand, or comparable document since 2014 regarding insurance carriers' non-compliance with mental health/substance use disorder parity laws and regulations

I also ask that all fees be waived for this request as I am a journalist employed by the Lowell Sun and I intend to use the requested records to publish articles in the public interest and not for any commercial purposes.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 10 calendar days, as the statute requires.

I am forwarding you the links to the 2014 report on the Division of Insurance website related to your request below. The 2014 information filed by the companies will be provided to you on a disc; the files are too large for our email system. Please let me now if you want the disc sent by USPS or if you want to pick it up. No documents relating to a violation, reprimand or comparable notice has been located for 2014. The 2015 information filed with the Division is currently under review and the report is not completed at this time. Please let me know if you require additional information.
