Pursuant to the Massachusetts Public Records Law, I hereby request the following records from:

1. All previous quarterly reports —those mentioned in section 652.07 of 103 DOC 652 (titled Identification, Treatment and Correctional Management of Inmates Diagnosed with Gender Dysphoria)— from the Gender Dysphoria Treatment Committee submitted to the Department of Correction’s Director of Behavioral Health regarding the Committee’s review of all cases of inmates diagnosed with Gender Dysphoria

2. All past annual budgetary documents demonstrating the amount of capital allocated to mental health services

3. A blank Treatment Plan form used by Primary Care Clinicians to outline the treatment plan for a person diagnosed with Gender Dysphoria

4. Any training materials used to inform Primary Care Clinicians about the needs of those diagnosed with Gender Dysphoria

Please provide all relevant materials for each of the following facilities:

- Lemuel Shattuck Hospital Correctional Unit
- Massachusetts Alcohol and Substance Abuse Center
- Massachusetts Correctional Institution - Cedar Junction
