At the direction of the Legal Services Department of the Madison Metropolitan School District as communicated to me on April 25, 2019, this request under the Wisconsin Public Records Law is being sent to "publicrecords@madison.k12.wi.us" instead of the email address for the district official who serves as Legal Custodian of Records under the district's Policies and Procedures 2615 established by the Board of Education.

Pursuant to the Wisconsin Public Records Law and the Madison Metropolitan School District's Policies and Procedures 2615, I hereby request the following records:

1. The following records pertaining to the Madison Metropolitan School District (MMSD)’s “Reading Drives Achievement: Procedural Compliance Self-Assessment” (RDA:PCSA) for the 2017-18 school year:

A. Records describing the systems of internal controls implemented by MMSD as corrective action for each RDA:PCSA section(s) found to be noncompliant;

B. All memoranda sent to staff (including regular education staff, if applicable) as corrective action for each RDA:PCSA section(s) found to be noncompliant;

C. All revisions to IEP form(s) implemented as corrective action for each RDA:PCSA section(s) found to be noncompliant;

D. All materials presented and/or distributed at department meeting(s) held to review procedures as corrective action for each RDA:PCSA section(s) found to be noncompliant, and records setting forth the names of attendees of such meeting(s);

E. All materials presented and/or distributed at in-service(s) conducted with staff (including regular education staff, if applicable) as corrective action for each RDA:PCSA section(s) found to be noncompliant, and records setting forth the names of attendees of such in-service(s); and

F. All revisions to written special education procedures implemented as corrective action for each RDA:PCSA section(s) found to be noncompliant.

2. All correspondence to or from MMSD, external and/or internal, pertaining to MMSD’s IDEA [Individuals with Disabilities Education Act] LEA [local educational agency] “Needs Assistance” determination for 2017-18.

As a reminder, if the requested records include any links to external websites (i.e. "live" links), all such links should be retained and not disabled in the records provided to fulfill this request.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

As provided by the Public Records Law, your response is requested as soon as practicable and without delay. If you choose to deny this request, please provide a written explanation for the denial, including a reference to the specific statutory exemption(s) upon which you rely.
