To Whom It May Concern:
Pursuant to the Massachusetts Public Records Law, M.G.L. c.66, §10, I hereby request the following records:
-A roster/inventory of all requests to the Pentagon's Program 1033. According to the DLA Disposition Services website, these applications are all sent to CAPTAIN THOMAS DONOGHUE or JOSEPH KELLY of the Massachusetts State Police. If a roster is not available, please provide copies of all applications.
-A roster/inventory of all items received from the 1033 program.
I also request that, if appropriate, fees be waived as we believe this request is in the public interest, as suggested but not stipulated by the recommendations of the Massachusetts Supervisor of Public Records. The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, processed by a representative of the news media/press and is made in the process of news gathering and not for commercial usage.
I expect the request to be filled in an accessible format, including for screen readers, which provide text-to-speech for persons unable to read print. Files that are not accessible to screen readers include, for example, .pdf image files as well as physical documents.
In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 10 business days, as the statute requires.

A file exists documenting our application to participate in the state 1033 program as well as an authorization letter for this program. The total file is 16 pages. Should you want to receive a copy of this file, please forward a check in the amount of $16.00 payable to the City of Lynn to Records, Lynn Police Department, 300 Washington Street, Lynn MA 01902 Attention Deputy Chief Santoro. . As soon as the check is received we will mail the documents to you. These documents may be provided to you “in hand” at a cost of .50 per page ($8.00) should you prefer to pick them up. Please let us know in advance if you intend to pick them up in person, so that the documents will be available for you.
We have received no items from the 1033 program, nor have we requisitioned any items from this program.
