Pursuant to the Virginia Freedom of Information Act, I hereby request the following records:

- A copy of any data release consent forms parents or students are asked to sign
- Any current policy documents or guidelines governing student data usage
- Any amendments made to those forms from 2015-2019
- Documents outlining any third-party programs or applications collecting student data

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

Good afternoon Mr. Nash,

We are in receipt of your FOIA request response email requesting additional
information under the Freedom of Information Act that states the following:
“ - A copy of any data release consent forms parents or students are asked
to sign - Any current policy documents or guidelines governing student
data usage - Any amendments made to those forms from 2015-2019 - Documents
outlining any third-party programs or applications collecting student data.”

In response, you will find the following information attached in the form
of a PDF document. Also although some of the documents do not require
signatures, we felt their inclusion may assist in answering your request.

1. Release of Student Data/Records (JOD): Lynchburg City Schools
policy and form regarding use of student data/records. There were two
policy updates during the time frame of 2015-2019 and the policy in use
will be the first one in the attachment dated 12.4.18 at the top right of
the page. The older version is dated and included in the attachment.

2. Student Records (JO): Lynchburg City Schools policy regarding
student records. There were four policy updates during the time frame of
2015-2019 and the policy in use will be the first one in the attachment
dated 12.4.18 at the top right of the page. The older versions are dated
and included in the attachment.

3. FERPA Notice: Notice to parents/guardians regarding FERPA. This
notice has been updated four times during the time frame of 2015-2019 and
the form in use is the first one in the attachment dated 11.30.18 at the
top right of the page. The older versions are dated and included in the
attachment.

4. News Media (KB-R) and Refusal of Permission/Media Opt Out Form:
Lynchburg
City Schools policy regarding news media and refusal of permission/media
opt out form. This version is currently in use and was updated on 2.18.14.

5. Internet Privacy (KBEZ): Lynchburg City Schools policy on internet
privacy updated 4.4.17 and is currently in use.

6. Administration of Surveys and Questionnaires (KFB): Lynchburg City
Schools policy regarding administration of surveys and questionnaires.
This policy was updated on 4.19.16 and is currently in use.

7. Acceptable Computer System Use (IIBEA/GAB): Lynchburg City Schools
policy regarding acceptable computer system computer use. There were two
policy updates during the time frame of 2015-2019 and the policy in use
will be the first one in the attachment dated 12.3.19 at the top right of
the page. The older version is dated and included in the attachment.
