Public Records Request Log - Louisiana Department of Children and Family Services

To Whom It May Concern:

Pursuant to the the Louisiana Public Records Act, I hereby request the following records:

A log of public records requests submitted to the Department of Children and Family Services that lists the date of request, the requester's name, the requester's organizational affiliation, a description of the request and the date the request was filled. Ideally I would like this information for the period from 2004 to the present.

The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, and is made in the process of academic research and not for commercial usage.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Some agencies I've received information from maintain a log for administrative purposes which is typically a spreadsheet that lists the names of individuals that submit requests to the department and a brief description of what the request concerned. If the Department of Children and Family Services keeps a log like that, then that's the only information I need.

Some departments don't maintain a log, if that's the case for the Department of Children and Family Services then I would like copies of all the PPR requests the department has received since January 1, 2004.

I realize this second option may incur some fees, if the department doesn't have some sort of log spreadsheet available could you please let me know any costs associated with getting copies of all the PPR requests for 2004 to the present?
