Pursuant to the Florida Sunshine Law, I hereby request the following records:

All records, reports, recordings and documents related to the arrest of

Scott A. Huminski in Bonita Springs at approximately 10:30 and the transport and drop off to the Lee jails including any video or audio recorded while en route from Bonita Springs to Fort Myers in a LCSO vehicle. The period of interest is from 10:00 A.M. to Noon on 2/12/2019 while Huminski was in custody of the arresting deputy.

Supply all documents, recordings, materials related to the compliance with the ADA (Americans with Disabilities Act) of LCSO transport vehicles (buses, vans, cars and other) that were used to transport Huminski both upon arrest and during other transport of Huminski that occurred during his stay at LCSO facilities.

Provide all documentation as to what ADA accommodations were provided to Huminski in his jail cell that allowed him access to the top bunk when the bottom bunk was occupied by another resident.

Provide all materials and documents related to Huminski's complete disability, medicare and social security and any and all materials related to ADA accommodations and treatment of the fully disabled at LCSO facilities (specifically to Huminski and generically).

All materials related to the determination that Huminski committed the crime of disorderly conduct at his intake as set forth in the records last produced by the LCSO and all materials related to LCSO procedures concerning placing a person in solitary confinement and disciplinary matters concerning inmates.

All handwritten or digital records related to the dispensing of medications to Huminski which include 15 to 30 notations of blood pressure pursuant to the directives of the LCSO medical staff including the blood pressure readings taken and recorded approximately 9 to 11 hours after arrest in Bonita Springs. Including Huminski's vitals which indicated a temperature of 97.0 after 8 + hours of solitary in inhumanely cold temperatures without adequate clothing or shelter. Prescribing directions from LCSO staff required the taking of Huminski's blood pressure at the time medications were dispensed, which was done and recorded by the LCSO staff that distributed medications. Supply this data.

Attached is a brief filed in the 2nd District Court of Appeals indicating LCSO knowledge of Huminski's disability situation discussed before LCSO staff in court.

All documents and materials related to the felony violations of the elder/disabled adult abuse statute perpetrated by the LCSO set forth in the attached emails to the Florida Department of Health that triggered 3 investigations of LCSO medical staff. See attached.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Please forward this matter to the FDLE and FL Attorney General's office for determination if the excessive delay in production of materials may be construed as criminal obstruction of justice concerning your behavior.

I seek assistance with getting compliance with the Sunshine Laws.
