Pursuant to the New Jersey Open Public Records Act, I hereby request the following records:

Please provide a copy of the Complaint I filed about Tina Booth with your agency, Monmouth Medical Center PESS.

Provide the following, previously requested in 2018 yet ignored by your office:

1. Provide Tina Booth's credentials, ie: Licenses, certificates, training, experience as of December 2, 2018
2. Provide Tina Booth's job description, when she was hired, who she reported to on December 2, 2018
3. Provide Tina Booth's relationship with the Manalapan Police Department.
4. Provide the date and time Tina Booth was first contact by any representative or law enforcement individual at the Manalapan Police Department.
5. Provide a detailed protocol for Home visits, policies and procedures.
6. Provide a copy of Tina Booth's resume and business card as of December 2, 2018.
7. Is Tina Booth required to provide a business card or identify herself in a first meeting?
8. Is Tina Booth authorized to break into a person's home and if so under what circumstances?
9. What is the criteria that Tina Booth utilizes to "diagnose" an individual?
10. Has Tina Booth received any formal complaints against her to your office and if so attach copies of those complaints and how your facility responded.

Please note that Lt. Thomas Mantle phoned my residence asking to stop by with a Property Loss Statement. After showing property vandalism and tree vandalism to a Manalapan Police Officer with Tina Booth broke into my residence. Tina Booth held onto the door handle to my front door as I closed it. She pulled the door back and there ensued a "tug of war" with Tina Booth breaking into my house. She was uninvited and asked to leave. She refused. The encounter was recorded.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
