Video recording of Trevin Lewis shooting

Dear Police Chief Jim Craft and/or Charita Benoit (Lafayette Police Department Records Supervisor):

Pursuant to the the Louisiana Public Records Act, I hereby request the following records:

1) immediate release of the complete video (in its unaltered form) of the police shooting of
Trevin Lewis that occurred on September 12, 2015 near Soulier and Sonny Sts. in Lafayette, LA.
This includes ALL digital video and images from the DVR confiscated from Ms. { } .

2) immediate release of the name of the police officer that shot Trevin Lewis

3) release of the personnel records of the police officer that shot Trevin Lewis

4) release police reports filed by the arresting officers from the Trevin Lewis shooting
investigation

The requested video and documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available, or either flash drive or CD-ROM if not.
