In order to proceed with the processing of your request, we require a signed affidavit that the records provided (list of persons) will not be used for commercial purposes of any kind. If we do not receive this form back and completed appropriately by August 31, 2020, we will consider your request abandoned and we will close the request. Also note that the information you're seeking is not available in one list/form and there is no requirement that we create a record that does not already exist. In addition, the date of birth (month/year) for criminal justice employees is exempt from public disclosure and inspection and will not be included. Reference RCW 42.56.250(8).
