Pursuant to the California Public Records Act (California Government Code §§ 6250), I hereby request the following records:

- A list of all police officer-involved shootings, arrest-related deaths and in-custody deaths reviewed or investigated by the District Attorney's office since 2004. Please detail the identity and agency of the officer(s) involved, the date and location of each incident, the decision reached by the DA's office, and any other available information about the encounters.

- Copies of all investigation reports on officer-involved shootings, arrest-related deaths and in-custody deaths reviewed by the DA's office or produced by the DA's office since 2004.

- Copies of all letters, memos or communiques sent from the DA's office to county law enforcement agencies notifying them of decisions reached by the DA's office on reviews of officer-involved shootings, arrest-related deaths and in-custody deaths since 2004.

Please send any responsive records to me as they become available rather than waiting for all elements to be releasable.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or DVD/CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 10 calendar days, as the statute requires.
