Pursuant to the Kentucky Open Records Act, I hereby request the following records:

Any copies of completed 'UAV Photography Application', 'Drone Application' or other documents or contracts regarding use of an unmanned aerial vehicle, unmanned aerial system, or drone, submitted by Matthew Warner in regards to the Identity Evropa conference held at Dale Hollow Resort State Park during March 2018. Please understand this request to specifically include *completed* and *filled-out* copies of the application, not a blank or 'draft' labeled empty version of the form.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
