Pursuant to Kentucky Open Records Act, I hereby request the following records:
I'm requesting a shapefile or csv (with Census Tract geoidentifier) with all Kentucky death statistics aggregated to the Census Tract level including Deaths by Race and Sex, Number and Crude Rate, Deaths by Age Group, Number and Age-Specific Rate, Leading Causes of Deaths, and a calculation of life expectancy and/or Years of potential life lost. I'm requesting these statistics be aggregated to the Census Tract level and include statistics for the most recent available year.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 3 business days, as the statute requires.

Good afternoon Patrick. Thanks for contacting our office at the Kentucky Department for Public Health for making your request. Let me make some clarifications for us to be able to provide support to your data needs.
- By statute, we are mandated by request, to share some information regarding our Kentucky deaths. The fields allowed by the law to be released are as follow: Name of the deceased, date and county of death. Any other pieces of information on this regard are protected and preserved in the Office of Vital Statistics (OVS), and it is under the state registrar's discretion the decision of releasing this other info, depending on the nature of the data request.
- Our death records are not currently available at a Census Tract Level nor geocoded. We have info at a county, district, and state level when aggregated, but the release of this information is subject to our Data Release Policy regulations.
- In the OVS, we don't have any info available regarding life expectancy and/or years of potential life lost. We may provide info regarding KY deaths by race, sex, age, and leading causes of death, upon registrar's approval. The most recent reliable year of data available would be 2014.
- We would need clarifications on the nature of this request, what is the purpose for having this data, and what will be your role on the manipulation of this data.
As we clarify all details on this request and we get the registrar's approval to proceed, then I will estimate the fee for the time needed for preparing this data for you. Please, let me know if you have any further questions or comments to this. I'll be looking forward to hearing from you.
