Pursuant to the Kansas Open Records Act (K.S.A. 45-215), I hereby request the following records:

Log of requests submitted under the Kansas Open Records Act to the Department of Health and Environment that lists the date of request, the requester's name, the requester's organizational affiliation, a description of the request and whether or not the request was ultimately fulfilled. Ideally I would like this information for the period from 2013 to the present.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 3 business days, as the statute requires.

However, federal and/or state law may limit or prohibit access to or disclosure of records or otherwise exempt records from public access or disclosure.

KDHE may charge and require advance payment of a fee for providing access to or disclosing records in order to recoup request fulfillment costs including, but not limited to, the cost(s) of staff time and computer services required to fulfill your request. You will receive notice if advance payment is required or if the cost(s) of fulfilling your request exceed(s) the initially estimated or requested fee.

I apologize for the delay in responding to your KORA request, the MuckRock website has the incorrect contact information for the Kansas Department of Health and Environment. Consequently, your emails will being sent to an email address that is not monitored daily. Once the emails were discovered last week, I attempted to inform MuckRock of their error and ask them to resend your request but have not heard back from them. I do hope this will reach you promptly.
