The data record layout, data frame, or any equivalent data framework used by this agency to document maternal deaths. Materials responsive to this portion of this request should include reference to the data elements collected by this department and should not include any personal information. Please feel free to redact any personally-identifying information.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

Program staff has had the opportunity to review and assess the KORA request you submitted via email. KDHE has determined your request may be partially granted. Attached please find the death data dictionary that the State utilizes.

In regards to the rest of your request, KDHE has determined that we do not possess the records you request. Moreover, as KORA does not require a public agency to create a record in order to respond to requests for information, your request will be closed. No fees will be assessed for this KORA request.
