I am requesting all Body-Camera videos from the below-listed incident. I also request all transport video of the below-listed incident. This should include all of the prisoner cage video.

Please also include the Arrest Report pertaining to the incident, if any.

Suspect Name: Olivia Claire Pagan
Charge: Probation Violation Warrant

I have attached the Web Search Result from the Jacksonville County Sheriff's Office showing that the Jacksonville Beach Police Department is the arresting agency in this case.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

Please provide a prompt acknowledgment of this Public Records Request.

Please email your response to the email-listed at the bottom of this Fax. Please reach out to the email below before mailing a response.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

I am sorry. I just re-read your email. I will be using an organization to mail the check on my behalf.

It might take them a week to do so, once they have the amount.

Is it possible for me to mail you a SASE now so that you have it ready to go once you have a payment? How many stamps does it need? Please provide an address for mailing.

Mr. Hodges,

For the "prisoner cage" are you referring to the holding cell? I'm not sure that there is video for those but will double check. We may have Sally Port video, I'll check with our IT person, if you are requesting that also.

As far as postage, I have no way of weighing the request, but I would guess 1.00 would be more than sufficient.

When the report is ready, please let me know, and I will have the organization I use to mail the payment.

You may receive the envelope before the payment. If that is the case, please just keep it until the report is ready to mail.

I have contacted the Postal Service to try and find where the package I sent to you is. I am going to withdraw this request at this time because I will not be re-sending the payment.

However, if the package does eventually arrive, I would appreciate you mailing me the report. If this is the case, and the postal service gets back to me, I will email you and let you know.
