Pursuant to the Kentucky Open Records Act, I hereby request the following records:

1. Please provide a copy of the Incident report taken by Sheriff Paul Hays May 2019.

2. Please provide copies of any photos taken.

To refresh your memory I reported the attempted break-in of my garage through the side door. The side door jam was pulled off the door frame likely with the crow bar left behind at the scene of the crime. There were also a set of keys left by the door that appeared as if someone attempted to first enter with key and when that failed used the the crow bar to remove the door frame. I incurred approximately $400 in damages. Sheriff Hays did not provide an Incident report number and I have not yet received the report.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
