Request for Iowa City School District FitnessGram Data Privacy Policy

To Whom It May Concern:

Pursuant to the Iowa Open Records Law (Code Chapter 22), I hereby request the following records:

Request Iowa City School District’s written contract or agreement with Human Kinetics regarding use, retention, sharing and use of student health data collected via the District’s implementation of the FitnessGram program. Any district documentation (including email) that permits or restricts Human Kinetics use of student data and the nature of those permissions or restrictions.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 10 business days, as the statute requires.
