Pursuant to the Iowa Open Records Law, I hereby request the following records:

The schema or data dictionary of all data tables used to compile records on COVID-19 cases, deaths and outbreaks at long-term care faculties. I am requesting a copy of the names of the tables and columns that store the data collected by the state, as well as any documentation used to describe and define the databases.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 10 business days, as the statute requires.

Dear Mr. Welsh. Please find the attached document and information in
response to your request for forms used to compile Covid-19 cases, deaths
and outbreaks at long-term care facilities.

A word document of the form used to collect information for all COVID-19
cases is attached. The form is formatted differently in Domo, but the
question that captures if a case works at a LTC facility is in the
'Work/School' section and if a case lives in a LTC facility is in the
'Household' section. Both sections have a question for the 'LTC Facility
Name' that is pre-populated with the Iowa LTC facilities. Screenshots of
the questions as they appear in Domo are provided below. All of the other
information captured on the form is the same for all COVID-19 cases.

All COVID-19 deaths are tracked by Vital Records. However, Vital Records
doesn't know that the decedent is a resident of a LTC when the death is
reported.

Unfortunately, we do not have most of the requested information in the
format in which it is requested, but have provided a few screenshots below.

There is an excel document and a card in Domo that are used to track LTC
outbreaks. The column headers are provided below. The excel spreadsheet
has a tab for the 'Current Outbreaks' and a tab for the 'Past Outbreaks'.
