Pursuant to the Indiana Access to Public Records Act, I hereby request the following records:

Absolutely any and all logs, reports, spreadsheets, databases, or other materials detailing all settlements or similar dispute resolutions to which this agency, or any division thereof, was a party, including, but not limited to, all conciliatory agreements related to complaints or court or legal proceedings against any representative of or office of this agency. Please include all available segregable data, including the entities involved, the date on which the initial complaint or court action was brought, the nature of the charge or complaint, the nature of the dispute resolution (i.e. in-court, out-of-court, in-agency mediation, third-party, etc.), any financial agreements associated with the settlement, and the date on which the case was closed.

Please provide all available materials from January 1, 2010 through October 31, 2017.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 7 business days, as the statute requires.

The Office of the Mayor has no record of receiving the November 6, 2017 public records request that was forwarded via fax today. We are not sure if the request was directed to the wrong fax number or there was some malfunction; unfortunately, that means we have nothing to report to you as requested.

Under Indiana law, specifically the Indiana Access to Public Records Act, IC 5-14-3-1, et seq., requests are to be submitted to the agency or agencies that maintain those public records-which from a quick look at what you were seeking, may be agencies other than the Office of the Mayor. Further, the APRA requires that public records requests be stated with reasonable particularity. The item you submitted by fax today is not reasonably particular as it is broadly stated (Absolutely any and all but no agencies/divisions of city government specified.)

For this reason, it is our recommendation that you submit this request via the on-line portal that has been set up for agencies that are part of the Consolidated City of Indianapolis/Marion County, Indiana. Submitting on-line will allow you to submit this request to specific agencies, including the Office of the Mayor should you choose. You will receive direct acknowledgment of receipt of requests you submit and direct communication from the responsible agency personnel concerning the request via email. If there are records responsive to this request, the agency can send those to you through the portal. You can either select Justice and Public Safety Records or Government Records, then select the agency that you wish to submit a request to-please note, an agency is under one or the other of these two categories so you may need to peruse both to find the agency you are looking for;

Once you find the agency, you can enter type of record and then complete the rest of the on-line form.

You will be asked to register-which can be done with an email address and a password of your choice.
