Pursuant to the Indiana Access to Public Records Act, I hereby request the following records:

Any results of, or correspondence (meeting notes, physical letters, email) related to, soil testing within a ten mile radius of the Exide Technologies facility in Muncie (2601 W Mt Pleasant Blvd, Muncie, IN 47302).

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 7 business days, as the statute requires.

Incomplete Public Records Request

The Office of Records Management (ORM) requires a form for public record requests. Please use State Form 55542, which specifies the information we need to conduct a search for responsive records. The form is attached in Word and PDF formats for your convenience. In addition, to request email communications, please provide enough information to allow IDEM to search for, locate, and retrieve the records requested. You should list the sender(s) and recipient(s), keywords or subject matter, and limit the request to a reasonable timeframe. Listing a specific entity and/or IDEM branch as sender and/or recipient may be sufficient for IDEM to locate and retrieve the records. Please be advised that the general retention period for IDEM emails is three (3) years, unless otherwise required under another retention schedule or pursuant to statute.

Please take time to complete and submit State Form 55542 via email to IDEMFILEROOM@idem.in.gov<mailto:IDEMFILEROOM@idem.in.gov>. We will presume that you are no longer interested in obtaining the requested information if we do not receive State Form 55542 (attached) completed in its entirety by 4:30 pm EST on Wednesday, November 28, 2018. IDEM will work diligently to fulfill your request. Please do not hesitate to contact the ORM if you have any questions.
